4-H Fair entry information. Click on a specific division below to find the corresponding fair entry form.

  • Please read the Fair Entry Instructions before filling out the entry forms.
  • All Fair Entry Forms are due to the Extension Office by July 1st.
  • 4-H Members must have been enrolled in the specific project area by May 1st to be eligible to exhibit that project at Fair.
