For all events and programs provided or supported by Oregon 4-H, accommodations related to a disability will be made where reasonable and possible.

Please use the form below to request an accommodation for a 4-H participant or volunteer. Return the form to the event director or your local Extension office and someone will follow up with you.

For events published on the Extension website, the accommodations contact is always listed with a phone number and email address. You may email this form directly to them.
