  • Banking and Financial Agreement: Please read over and sign this document in order to comply with non-profit, charitable corporation rules when opening and maintaining a bank account for your 4-H club or committee.

  • Financial Gift or Donation: Oregon 4-H clubs and Associations are recognized as not-for-profit organizations. Please use this form if your 4-H club or committee receives a financial gift or donation.

  • Financial Gift or Donation Guidelines: This resource details out best practices guidelines when your 4-H club or committee receives a financial gift or donation.

  • Request to Conduct 4-H Fundraiser: Please use this form to submit a request if you are planning a fundraiser for your 4-H club or committee. Your request must be approved prior to advertising, promoting, or holding the event.

  • State Committee Application: There are several advisory committees that help guide the various programs within 4-H. Each committee is different in terms of number on the committee, how often they meet, and how long the term of service is.
